I suspect it would work well as a matrix -- general level of anxiety, sleep/other physical effects, resistance to behaviours that might lead to anxiety/other avoidant behaviour, panic attacks. Or something along the lines of the short (15 question) depression scale I can't remember the name of, where it is the overall degree of impairment, and not the specific patterns of impairment.
