That's okay. Often conversation with other folks can help to refine things like this. I've touched up my Safety Tracker a few times. However, your current version of the Anxiety Scale looks highly usable. Folks might need to tweak it for personal details, but in general, it seems very promising and I haven't seen a similar thing elsewhere.
